A List of Participating Mortuaries as of March 2017

Direct Cremation

Professional service charge, cremation, securing legal documents (use of vehicle), transportation from place of death: 30 mile radius. Beyond 30 miles current cost to mortuary.

Not included: Simple cremation does NOT include cost of permit to cremate, memorial or cemetery charges. Next-of-kin may have the cremated remains at no added charge.

Direct Burial (Without Memorial Service)

Professional service charge, simple flat top cloth-covered casket, securing legal documents (use of vehicle), transportation from place of death: 30 mile radius. Beyond 30 miles, member pays current cost to mortuary. Transportation to cemetery (not a funeral coach), 30 mile radius.

Not included: Simple burial does NOT include permit to bury, memorial service or cemetery charges.

Simple Burial with Funeral Service (All Faiths)

Professional service charge (embalming, viewing Chapel or graveside service and all necessary preparations), simple flat top cloth-covered casket, securing legal documents (use of vehicle), transportation from place of death: 30 mile radius. Beyond 30 miles, member pays current cost to mortuary. Transportation to cemetery (not a funeral coach), 30 mile radius.

Not included: Simple Burial with Memorial Service does NOT include funeral procession, clergy, limousine, motorcycle police escort, music, flowers, newspaper notice or cemetery charges. Funeral procession requires a funeral coach. Visitation/Rosary, and/or Funeral or Memorial Service to be during the week for a maximum of 3 hours starting after 9 am and concluding before 4 pm. A graveyard service in lieu of a chapel service, is to be during the week between the hours of 10 am and 3 pm. An extra charge will be made for weekend or after hours.

There is an additional charge for death certificates from the State of California.

PLEASE NOTE: A recordkeeping fee of $25.00 will be collected by the mortuary and sent to LAFS for each member serviced. This is a part of the membership fee charged at the time the mortuary is used.

VITAL INFORMATION
Social Security Death Benefits: There is a one-time $255.00 Social Security death benefit available under certain conditions. In general, it applies to minor children and to adult disabled children, and to a surviving spouse of deceased. Check with Social Security at (800) 772-1213 as soon as possible after death occurs.

Veteran Death Benefits: A veteran would not be eligible for any benefits if he/she were not drawing benefits or did not die in a Veteran’s facility. They WOULD be entitled to a plot and headstone in a national cemetery. Los Angeles Veterans Cemetery has no space left, but there is space in Riverside and Bakersfield Cemeteries for cremations and burials. To apply for any of these benefits and for further information, call (800) 827-1000.

When There Is No Money: For persons considered indigent by the L.A. County Public Administrator, arrangements can be made for cremation and burial in a common grave through the mortuary at LA-USC Medical Center, 1200 N. State St., L.A., (323) 226-2622. For the family to receive the cremated remains, they must pay the cremation charge.

Willed Body Programs: To will your body to medical science call the Dept. of Anatomy at UCLA, 310-794-0372. The Anatomical Gift Program at USC will also accept whole body donations. Call USC at (323) 222-0231. The schools will pick up the body. It will be kept for a few years, at which time the County will cremate with other bodies. This will be at no cost to the family, except for mileage over 50 miles ($1.50 per mile) and the State charge of $16.00 for the death certificate. They will not accept a body which has been autopsied or embalmed. They need to know at the time of donation if the body is to be cremated by the County. If not, you may make your own arrangements at your own expense.

Coroner: The Coroner is involved when death is the result of other than natural causes or the deceased has not been seen by a doctor for 6 months.

This is part of our ongoing commitment to you of constant vigilance of the mortuaries with whom we are associated. New Mortuaries are investigated thoroughly before we negotiate a new contract. We are proud of our record – and our mortuaries.
